To send permission:
In order for a child under the age of 13 to become an official member of this site, we will need a signed letter of permission from the child's parent or guardian. This letter must include:
- Parent/Guardian's full name, email address, and mailing address
- Child's full name, email address, and mailing address (if different from parent/guardian's)
- Statement of permission similar to the following: "I/We, the parent(s)/guardian(s) of [child's name] give our permission to become an official member of DisciplesNet Church and to use the DisciplesNet website. I/We understand that DisciplesNet will keep the information provided in this letter to verify our permission.
- Signature of Parent/Guardian
This letter can be mailed to the DisciplesNet office (address below) or a scanned copy can be sent via email to either pastor@disciplesnet.org or mailinglist@disciplesnet.org.
After the letter is received and filed, we will send parent and child an email with the child's username and password for logging on to DisciplesNet Church as an official member.
While our website is appropriate for all ages, products and services are all directed to people who are at least 13 years old or older.
Only specific people have access to our user information, including information for users below the age of 13. Those people are currently Senior Pastor Deb Phelps and Associate Minister Russ Smith.
